Research

Dr. Martin’s research interests are focused sports injuries of the hip, knee, and shoulder. The majority of his research is dedicated to optimizing management of joint injuries either surgically or by physical therapy with the goal of joint preservation. He is involved in a number of ongoing studies and has, in particular, concentrated on optimizing treatment for patients with labral tears of the hip and developing novel surgical techniques for hip preservation. Other areas of interest include rotator cuff injuries and ligamentous injuries of the shoulder, labral tears, femoroacetabular impingement, gluteal tears of the hip, and complex and isolated knee ligament reconstruction.

He has been involved in large multicentered clinical trials such as the METEOR trial that compared surgical correction of knee meniscal tears to physical therapy in patients who also had knee osteoarthritis. His current studies include clinical trials, prospective patient databases, retrospective chart reviews, and anatomic investigations.

Dr. Martin collaborates with the Harvard Medical School Tissue Engineering Laboratories in several projects. His most recent projects concern the use of biologic treatments such as stem cells to optimize management of sports related injuries of joints.

Comparing the Risk of Osteonecrosis of the Femoral Head Following Intra-articular Corticosteroid and Hyaluronic Acid Injections, JBJS

Conclusions: The incidence of ONFH after intra-articular hip injection was similar between patients who received CSIs and those who received HAIs. Although this study could not determine whether intra-articular injections themselves (regardless of the drug that was used) lead to ONFH, the results suggest that ONFH after CSI often may be due, in part, to the natural course of the underlying disease. Future randomized controlled trials are needed to definitively answer this question; in the interim, clinicians may be reassured that they may continue judicious use of CSIs as clinically indicated.

Conclusions: Patients with moderate cartilage injury undergoing arthroscopic acetabular labral repair with BMAC application reported significantly greater functional improvements when compared with similar patients without BMAC application.

Minimum 2-Year Functional Outcomes of Patients Undergoing Capsular Autograft Labral Reconstruction, Am J Sports Med

Conclusions: In this study of 97 hips undergoing arthroscopic labral repair with capsular autograft labral reconstruction, we found favorable outcomes that exceeded the MCID thresholds in the majority of patients at a mean 28.2 months’ follow-up. Future studies should compare outcomes between this technique and other methods of autograft and allograft reconstruction to determine differences in patient-reported outcomes, donor-site morbidity, and complications.

Hip Arthroscopy versus Physical Therapy for the Treatment of Symptomatic Acetabular Labral Tears in Patients Older than 40: A Randomized Control Trial, Am J Sports Med

Conclusion: In patients older than 40 years with limited osteoarthritis, arthroscopic acetabular labral repair with postoperative physical therapy led to better outcomes than physical therapy alone. Thus, age over 40 years should not be considered a contraindication to arthroscopic acetabular labral repair.

Hip Joint Preservation Technique

Use of Bone Marrow Aspirate Concentrate with Acetabular Labral Repair for the Management of Chondrolabral Junction Breakdown

By using BMAC to augment labral repairs and coat chondrolabral junction breakdown, we are able to introduce mesenchymal stem cells to peripheral acetabular tissue with little to no drawbacks, while avoiding donor-site morbidity, open procedures, and multiple surgeries.

Accelerated Versus Standard Physical Therapy in Patients with Transtendinous Rotator Cuff Repair: A Propensity-Matched Cohort Study, J Shoulder Elbow Surg

Conclusion: Accelerated PT following transtendinous RCR is associated with significant improvement in AROM at 6 weeks and 3 months postoperatively. Further, the early motion may help obviate the development of severe postoperative stiffness without any evidence of higher rotator cuff retear rates.

Worsening Racial Disparities in Patients Undergoing Anatomic and Reverse Total Shoulder Arthroplasty in the United States, J Shoulder Elbow Surg

Conclusion: Racial disparities in patients undergoing TSA and RTSA are worsening over time. Black patients undergo TSA and RTSA at lower rates than white patients regardless of insurance status and have increased odds of complications and mortality. Improved initiatives are needed to reduce these racial disparities and further research is warranted to understand their root causes.

Conclusion: DIME is highly sensitive for supraspinatus FTTs in patients with negative traditional rotator cuff PE tests for whom there is still high clinical suspicion of FTTs. Thus, this test is an excellent secondary screening tool for supraspinatus FTTs in patients for whom clinicians suspect rotator cuff pathology despite negative traditional static PE tests. Given its high sensitivity, a negative DIME test rules out supraspinatus FTT well in these patients, and can therefore better inform clinicians which patients should undergo MRI. In addition, the angle at which patients first report pain on unopposed active shoulder abduction is highly specific for supraspinatus pathology.

Conclusions: The use of MRI before a trial of conservative management in patients with atraumatic shoulder pain, minimal to no strength deficits on physical examination, and suspected cuff tendinopathy other than full-thickness tears provides negative value in the management of these patients, at both the individual and population level.

Conclusion: Readmission after ACL reconstruction was 1.2%. The strongest risk factors for 90-day readmission include cerebrovascular disease, diabetes mellitus, concomitant MCL/LCL reconstruction, concomitant PCL reconstruction, and age between 44 and 65 years. Readmission was associated with substantial health care costs.

Although symptoms of clicking and intermittent locking had a greater reduction in the APM group, the presence of “meniscal symptoms” in isolation should not inform clinical decisions surrounding APM vs. PT in patients with meniscal tear and knee OA.

Conclusion: This cohort of patients with meniscal tear and osteoarthritis showed marked advancement in MRI-based features over 18 months. Patients treated with APM showed more advancement in some features compared to those treated nonoperatively. The clinical relevance of these early findings is unknown and requires further study.
